FPS Medical Center is a family practice that models itself after a patent centered philosophy called the Medical Home. The Medical Home is best described as a model or philosophy of Primary Care that is patient-centered, comprehensive, team-based, coordinated, accessible and focused on quality and safety. It has become a widely accepted model for how Primary Care should be organized and delivered throughout the health care system.
It is a philosophy of health care delivery that encourages providers and care teams to meet patients where they are, from the simplest to the most complex conditions. It is a place where patients are treated with respect, dignity, and compassion. Above all, the Medical Home is not a final destination. Instead, it is a model for achieving primary care excellence so that care is received in the right place, at the right time, and in the manner that best suits a patient’s needs.

I finished my undergraduate degree in Public Health and Community Medicine in 1975 and obtained my Medical Degree in 1979 at the University of Washington in Seattle.
I moved back to Arizona and completed my Family Practice Residency at Scottsdale Memorial Hospital in 1982. In 1985 I established FPS Medical Center.

What type of insurance do you accept?
We accept all major insurances including Medicare and Access.
What should I bring to my office visit?
To make the most of your visit, it’s best to bring the following to your appointment:
For Current Patients:
- Completed any forms we’ve sent to you in advance
- Insurance information for the patient
- List of current medications the patient is taking
- Documentation from any recent emergency room visits
For new patients:
- All of the items in the current patient section
- Past 2 years of medical records
What if I have to reschecule my visit?
Please call FPS Medical Center immediately at 928.855.1550 to cancel your appointment. We ask that you cancel at least 24 hours in advance. To reschedule an appointment, please call the office as soon as possible.
Can I call in my refills?
Medication refills are done at each office visit and can’t be done over the phone.
